Patient Journey
For most commonly prevailing chronic conditions viz. Diabetes, Hypertension and Hyperlipidemia, Novitas has simplified the entire patient journey by making quality care available at patients’ doorstep.
For rest of the chronic conditions, patient journey shall be prescribed depending on the stage of chronic condition and other parameters.
1-to-1 conversation between patient and one of our specialized physicians to know about patients’ lifestyle and concerns which you may be facing regarding your health and a brief history of the chronic condition.
Schedule Consultation
Once the one-to-one call with physician is done, Novitas takes a note of all the important dates for patients’ consultation, laboratory and medicine supply.
Novitas shall remind the patient 2 days in advance about the consultation.
If prescribed for any laboratory investigations by the doctor, Novitas shall request patient for the his/her availability.
Upon confirmation, a care giver is arranged to visit the patient and collect the sample. Necessary instructions on fasting or follow up shall be sent by text as well as call.
